Remembrance Submission Form

The pandemic has resulted in many forms of collective and individual loss. As a university community, we acknowledge in particular the loss of loved-ones due to and during the COVID-19 pandemic. We will use the information provided in the form below to create and display temporary memorial signs on campus, helping us all to honor their memory together.
